Membership Agreement

By completing this form, I understand that:
1 I agree to pay the annual subscription as set by BPW Gisborne Executive at the time of this application.
2 I am now booked in for all monthly dinner meetings automatically.
3 If I am unable to attend a monthly dinner meeting, I am responsible for submitting an apology.
4 If I do not submit an apology and do not attend the dinner meeting, I am responsible for dinner/meeting fee.
5 I agree to my image being used on the Gisborne BPW social media sites and in other publicity from time-to-time
